Neurological and Spinal Surgery, L.L.C. participates in a number of insurance programs and managed care plans. Please bring your insurance card with you so that we can work with your insurer to obtain the highest possible reimbursement for you. Precertification
Many insurance companies or providers require pre-certification for certain services; such as x-rays, MRIs, CT scans, injections, etc. Our office will assist you in obtaining any required pre-certifications.
Prior Authorization
Many HMO and PPO plans require that all medical services be authorized by your primary care physician. If your plan has such a requirement, it will be your responsibility to obtain all referrals or authorizations prior to visiting or receiving services from Neurological and Spinal Surgery, L.L.C.
Worker’s Compensation
If your visit is a Worker’s Comp visit, you must have prior approval in order to be seen.
Surgery/Procedure Pre-payment
You will be contacted by our staff prior to your procedure to discuss pre-payment on your estimated out of pocket costs. Pre-payment is required for elective procedures. Please refer to our Financial Policy for further information.
Co-Payments
If your insurance company requires that a co-payment be collected by us prior to your visit, please come prepared to pay your co-pay amount. If office visits are applied to your deductible and your annual deductible has not been met, you will be required to pay $100 at check-in to be applied to your deductible. You will be balance billed for any remaining amount.
If you are a “self-pay” patient, please be prepared to pay for your office visit on the day you come in to see our healthcare provider.
